Lead Hazard Tenant Registration & Inspections

All owners of residential buildings constructed prior to 1978 and that are rented for residential use, or have residential rental dwelling units, must complete this form. Each individual residential dwelling unit requires its own form to be completed. The form requires tenant information and lease information for each residential rental dwelling unit. The form also requires that the owner either upload their own Lead-Safe Certificate obtained after a Lead Hazard Evaluation conducted by a firm hired directly by the owner, or for the owner to request a Lead Hazard Evaluation from the Board of Health inspection firm.
